If your student has been accepted to Mountain Montessori you now need to submit student registration forms 1 thru 5.

 

When completed the necessary PDF records will be generated that must be on file for you and your child by the State of Colorado.  Click each link on this page and submit the information online.

  • STUDENT REGISTRATION FORM
    If you have been accepted to Mountain Montessori you will need to have this registration package on file with the school.
  • STUDENT SOCIAL HISTORY FORM
    This provides the school with important information and routines as it relates to your child.
  • STUDENT MEDICAL CARE FORM
    This form provides the school with important emergency medical contact information.
  • SCHOOL WAIVER
    This is to acknowledge that Mountain Montessori utilizes a full range of Montessori materials in our classroom to offer an authentic Montessori education.
  • SCHOOL POLICIES & PROCEDURES AGREEMENT
    Parents must agree to the rules that apply to all students and their families and will abide by these policies and procedures.
